Full-time Professors (Staff, contracted personnel)

1.     Complete the basic personal information form (collected from the human resources department or downloaded below).

2.     A copy of degree certificates (including Bachelors, Masters, Doctorate).

3.     A copy of teaching certificates (Lecturer, Assistant Professor, Associate Professor, Professor). Staff and contracted personnel are exempted.

4.     A copy of the resignation certificate from the previous employer .

(This includes a resignation certificate issued by the public or private school for full-time employment before joining our University, with a note indicating whether pensions have been received). Those without it are exempted.

5.     A copy of the industry resignation certificate.

(Resignation certificate for employment in the industry before joining our University). Those without it are exempted.

6.     A copy of certificates and licenses. (This includes various types of certificates and licenses for Levels A and B, professional skills certificates, Certificate of High Level or the Ordinary Level Civil Test, etc.).

7.     A copy of the Military Discharge Order. Females are exempted.

8.     Five 2-inch color photos.

9.     A copy of your Yuanta Bank account number.

10.   Notification Statement on Personal Information Collection.

 

Adjunct Professors

1. Complete the basic personal data form.

2. A copy of the highest degree graduation certificate obtained.

  (Professors with a foreign degree shall submit a copy of the highest education certificate obtained in Taiwan and a copy of their foreign education certificate).

3. A copy of your teaching certificate. (Please attach a copy of teaching certificates for an associate degree or above). Those without it are exempted.

4. A copy of your Yuanta Bank account number.

  (For those without an account, please visit a Yuanta Bank branch to open an account).

5. Certificate of Employment.

6. A completed copy of Survey on the Current Status of Adjunct Professors.

7. Notification Statement on Personal Information Collection.

8. Criminal Record Certificate issued by the Police Department.
